Patient & Family Care

The Interdisciplinary Palliative Care Approach involves specialised medical, psycho-social, spiritual and bereavement care to the patients and their families in the comfort of their homes. The focus is on the reduction of total pain to improve the quality of life for both the patients and their families.

Palliative care at St Luke’s Combined Hospices is provided to adult patients from the age of 18+, who have diagnosed with life-limiting and life-threatening illnesses like:

  • Cancer
  • End-stage incurable heart, lung, liver, kidney diseases
  • Neurological diagnosis such as Motor Neurone Disease
  • HIV/AIDS

Excludes the following diagnosis:

  • Stroke
  • Alzheimers
  • Dementia
  • Psychosis
  • Alchohol / Drug withdrawal

Palliative Care entails:

  • Prevention and relief of suffering through early identification, holistic assessments and management of total pain
  • Affirmation of life and regard dying as a normal process
  • Integration of spiritual care regardless of faith, culture or belief system
  • Supporting patients and their families with bereavement and counselling services

Ongoing-Care at Home includes:

  • Full access to the Interdisciplinary Team
  • Educating and supporting families with managing the care of the patient
  • Facilitating drafting of wills, social security applications (disability, pension, grant in aid and ID), school applications and UIF applications
  • Working in collaboration with other community based services

Out-Patient Community Based Palliative Care

The community based service is available and accessible to registered patients who are independently functional and mobile. There are a total of 9 community day hospices situated in Athlone, False Bay, Grassy Park, Gugulethu, Khayelitsha, Mitchells Plain, West Coast and Kenilworth

Community Day Hospices offer:

  • Full services of our Interdisciplinary Team
  • Support groups for patients
  • Personal enrichment and development activities
  • Respite for patients and their families
  • Interactive educational activities, debriefing and fun outings

Community Hospices

The Community Hospices offer an opportunity of support, socialisation and relief for care givers. Patients join activities once per week and also have access to the services of the interdisciplinary team consisting of nurses, social worker, volunteers, spiritual counsellor and medical doctor depending on their individual needs. This model increases the quality of life where patients are supported and prevents social exclusion or feelings of isolation. Fun in-house and extramural activities are arranged to engage and stimulate patients in a meaningful way.

How do I access care?

The referral process can take time and there is most likely a waiting list. Therefore, contact with our team should preferably be sooner rather than later. It is important to understand that first contact with St Luke’s Combined Hospices should not be as an ‘emergency service’. Rather it is a considered, and long-term, holistic care service designed for the specific needs of each individual patient and their family. Care is provided at no cost unless you are a member of a Medical Aid where the medical aid rates will apply for your specific medical aid plan.
